Clinical Case
Alexander, 67, presented to Yarema Dental with complaints of severe tooth mobility, gum inflammation, unsatisfactory smile esthetics, and an inability to properly bite and chew food.
During the diagnostic examination, significant pathological tooth mobility, periodontal pockets up to 7 mm deep, impaired masticatory function, and an unfavorable prognosis for the long-term preservation of the remaining teeth were identified.
Following CBCT diagnostics, photographic documentation, digital scanning, and occlusal analysis in an articulator, the Yarema Dental team planned a full-arch rehabilitation of both jaws.
The treatment plan included extraction of all teeth with an unfavorable prognosis, placement of five Straumann BLT implants in the upper jaw and six implants in the lower jaw.

The available bone volume was sufficient for implant placement without extensive bone augmentation. Augmentation was performed only locally in the extraction socket areas.
Temporary Restorations
Seventy-two hours after the surgical stage, temporary fixed full-arch PMMA restorations on a bar-supported framework were fixed for the patient.
This protocol allowed the patient not to remain without teeth during the healing period and restored esthetics and basic masticatory function.
At the same time, the patient was required to follow recommendations regarding diet, oral hygiene, and limiting the load during osseointegration.

Osseointegration
The osseointegration period lasted eight months.
At the follow-up stages, clinical examinations were performed, the condition of the soft tissues, occlusion, hygiene, and implant stability were assessed, and follow-up CBCT diagnostics were performed.
Occlusal Rehabilitation
The patient had an Angle Class II occlusion.
During the prosthetic rehabilitation, the vertical dimension of occlusion was increased by approximately 3 mm.
This decision was tested using the temporary restorations, assessing the patient’s adaptation, masticatory function, speech, and occlusal contacts.
Definitive Restorations
After completion of osseointegration, definitive full-arch restorations were fabricated from monolithic zirconia on titanium frameworks.
In the anterior region, individualized ceramic layering was performed to achieve a more natural transmission of color, translucency, and tooth anatomy.
The definitive prosthetic stage lasted approximately one month.

Treatment Outcome
As a result of the treatment, the patient received fixed implant-supported restorations, restored masticatory function, a corrected bite, and a more harmonious esthetic appearance of the smile.
The patient is satisfied with the treatment outcome.
Treatment performed by the Yarema Dental team.
